Submit an Obituary

You and your family members are invited to create a free obituary for a loved one that will be published on the Niagara Hospice website. Please fill out the form below and attach a photo if you would like. To ensure you don’t lose any of your writing, we recommended typing the obituary and saving it first in a word processing document, and then copying and pasting it into the form below.

Once you have completed all of the sections and added any service information, just click the submit button and it will be published soon. We recommend submitting your obituary at least two days prior to the service to ensure time to publish it and share online. If you have any questions, please call us at 716-439-4417.
