SOCIAL WORKER
Full-time MSW position available

Unique Hospice Social Worker position available. Successful MSW candidate will posses all necessary skills required to help patients and families prepare for life's final journey. Patient caseload will include both homecare patients and approximately 22 nursing home residents residing on a unit dedicated to providing hospice services. Familiarity with Long Term Care Medicaid or the ability and willingness to quickly learn the process is required. Some nights, weekends, holidays; travel throughout Niagara County required.
