Falls Prevention Coordinator: APPLY NOW!
Recruiting a Physical Therapy Assistant to coordinate falls prevention program for corporation. Coordinator will develop an evidence-based falls prevention program that includes components of identifying patient risks for falls. Work with each company to identify risk, monitor compliance, and maintain statistical reports to include both community and in-house patients. Travel required.

Niagara Hospice is recruiting an Advanced Practice Registered Nurse(Nurse Practitioner) to compliment its medical team. Duties include but are not limited to performing comprehensive patient assessments, using evidence-based research data in clinical decision making, establishing and documenting appropriate diagnosis, treatment planning and strategy of care based on the individual assessment, provides expert guidance and education when working with clients, families and other members of the hospice team.  The NP will work closely with the medical team to provide compassionate hospice care in all settings of Niagara Hospice; Niagara Hospice House, skilled nursing facilities, hospitals, homes and other locations. Must possess a current Nurse Practitioner certification/license, minimum of two years NP experience in geriatrics, in-home care or hospice. Excellent leadership, interpersonal, written and verbal communication skills required.
