Who will make health care decisions for you if you are unable to?

November is National Hospice and Palliative Care Month. To help raise awareness of health care choices, Niagara Hospice will host an Advance Care Planning program that is free and open to the community. Everyone age 18 and older should designate a Health Care Proxy. Learn what a Health Care Proxy is and what you need to do to make sure your health care wishes are followed. Join Niagara Hospice Medical Social Worker Cheryl Ferguson, BSW, MSW for this program.
